REGISTERING WITH THE PRACTICE
Our doctors need the correct medical records for you. In order for these to be traced we will need to know your full name, date of birth, current and previous addresses and previous doctor. You will be asked to fill out a registration form (GMS1) or to supply your medical card. This applies to each member of your family.
You will also need to produce two supporting documents to confirm these details, one of which MUST confirm your address is within our catchment area.
- Provide supporting evidence of identity (for each individual over 18), i.e. National Passport, Photo Driving Licence, National Identity Card, Application Registration Card (ARC) and visa where applicable. OR
- Personal bank/credit card, National Insurance card, NHS Medical Card AND
- Evidence that you are ordinarily resident in the UK and have been for 6 months (per family), i.e. utility bill, tenancy agreement, bank statements, mortgage statements, council tax bill or other official/government correspondence verifying proof of address. If you have just come into the UK we will need proof that you intend to work or study in the UK.
- Work Permit or supporting letter from your employer or the education provider.
To be accepted onto the practice list you will need to arrange your new patient health check (over 16’s only). This should be booked at the time of registration and completed within 4 weeks of registration. Your registration forms take 48 hours to process and your health check should normally be completed before you may book your first appointment to see a doctor.
On receipt of documentation showing proof of residence and identity and completion of your health check, we will fully register you with the surgery, subject to the Practice list being open. If you are unable to provide proof of residency and identity, you may be refused registration.

REGISTRATION FORMS
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file below, which you may print out and bring with you come to the surgery to register.
