| REGISTERING WITH THE PRACTICE
Our doctors need the correct medical
records for you. In order for these to be traced, we will need
to know your full name, date of birth, current and previous
addresses and previous doctor. You will be asked to fill out a
registration form (GMS1) or to supply your medical card. This
applies to each member of your family.
You will also need to produce two
supporting documents to confirm these details, one of which MUST
confirm your address is within our catchment area.

Provide supporting evidence of identity (for each individual over 18) : i.e.
national passport, photo driving licence, National Identity Card,
Application Registration Card (ARC) and visa where applicable

- Personal
bank / credit card, National Insurance card, NHS Medical Card.

| AND |

Evidence that you are ordinarily resident in the UK and
have been for 6 months (per family). i.e. utility
bill, tenancy agreement, bank statements, mortgage statements,
council
tax bill, or other official / government correspondence verifying proof
of address.

You will need to have a
New Patient check with one of our Practice Nurses before being fully
registered. This gives you an opportunity to see the practice, ask any
questions and for us to get to know you. It also enables your doctor to
obtain important information on your health needs and to make some
simple but important checks.
PRACTICE AREA
Our practice has a
defined boundary which has been agreed with the Primary Care Trust.
Please check with us to ensure that you live within this boundary
before your register. If you move, please check that you are
still within our practice area, as the boundary may have changed
since you originally registered with the practice.
If you are outside the
boundary, you will be asked to find another doctor.
Please also do make sure we have
your current address and phone number.
HOW TO SEE YOUR DOCTOR OR NURSE
Book an appointment by ringing: 01737 642207
Monday to Friday between 8.00am and 6.30pm.
Or call into the surgery:
Monday to Friday between 8.00am to 12.30
or 2.00pm to 6.30pm.
If you can't keep an appointment, please let us know. Thank

APPOINTMENTS

Appointments are booked at 10 minute intervals

A number of appointments can be booked up to 4 weeks in advance, but
most are made available on the day

For routine consultations, we will try to offer you an appointment
within 2 working days. For urgent medical requests, we will offer an
appointment on the same day. If you wish to see a doctor of your
choice, you may have to wait longer. The
receptionist will make every effort to help.
MISSED APPOINTMENTS
Due to the number of patients failing to attend for their
appointment this may mean that you may not be able to see the doctor on
the day that you wish to. In an attempt to try and resolve this, the practice has
developed the following policy:
If you fail to attend appointments without informing us we
will write to you asking if there are any specific problems preventing
you from letting us know.
If you repeatedly fail to attend for appointments you may be
removed from the practice list and have to find an alternative GP
practice.
HOME VISITS
If you are too ill to
get to the surgery, then a doctor will come to your home.
Transport problems are not a reason for requesting a home visit.
If you need a home
visit, please ring before 10 a.m.
REQUESTING A REPEAT PRESCRIPTION
Prescriptions will be ready for collection 2 full working days
after they are received by us. If you want your prescription returned
to you by post, you may provide us with a stamped, self-addressed
envelope. The two local chemists also provide a collection service
WEEKEND AND OUT OF HOURS SERVICES
If you need a doctor out
of hours, you should call the surgery number - 01737 642207 - and
listen to the instructions. You can then choose to be put
through to NHS Direct or to Thamesdoc, our deputising service.
Alternatively, you may visit the Walk-In Centre at East Surrey
Hospital.
